Concierge Pharmacy Services
A program designed to help you manage your medications and reduce the cost of medication therapy by personalizing your medication therapy for you. During the initial evaluation we document your medications, drug allergies, health conditions and identify potential drug interactions. Next we evaluate your current therapy and recommend adjustments if necessary. Each patient receives a personal medication record and a a personalized medication action plan.
This is the follow-Up visit that occurs after the initial Medication Therapy Management Evaluation. During this visit we check up on your progress since the last visit.
Sometimes a medication is working but your regimen may need a slight adjustment. With medication tailoring we will fine tine a your medication regimen to work a little better for you. Think of this a the version of the MTM initial evaluation that focuses on resolving a Specific Medication Issue

A 30-minute Individual nutrition counseling assessment includes a review of anthropometrics, biochemical, clinical and dietary assessments. This assessment is used to address specific nutrition related issues and topics. This data builds a foundation for the personalized nutrition intervention. Following the assessment is the Nutrition counseling portion of the session where we discuss the ways to modify diet and lifestyle to achieve targeted client outcomes.
